Authors from the University of Toronto take a closer look at medicine reconciliation in this Healthcare Quarterly article. They believe that the medicine reconciliation process is all too often regarded as a superficial administrative accounting task and argue that to reduce the harm caused by medication errors hospitals should invest in a more robust and effective strategy.
