Medication reconciliation during transitions of care as a patient safety strategy

This review, published in the Annals of Internal Medicine, summarises evidence about the effectiveness of hospital-based medication reconciliation interventions. Eighteen studies evaluating 20 interventions were included.

The review finds that medication reconciliation alone probably does not reduce postdischarge hospital utilisation but may do so when bundled with interventions aimed at improving care transitions.
