Primary care improvement case study: Nelson Marlborough Health: Improving medication adherence post cardiac stent

As part of a Whakakotahi improvement project, Nelson Marlborough DHB, Mapua Health, Tima Health, Harley Medical and other primary care providers, started a project called ‘Live longer and feel better following treatment for a heart attack’. It focused on the reduction of preventable deaths and acute readmissions to hospital following an angioplasty procedure. The improvements focused on improving the uptake of triple therapy (medication) following angioplasty and the reduction of unnecessary polypharmacy one year after this procedure.
