Aotearoa New Zealand has an excellent health care system that provides safe and efficient care to people using its services. However, harm can still occur due to failures in that system.
At the centre of every event of harm there is a consumer, their whānau and the health care workers involved. Providers have a responsibility to review, learn and implement effective changes to the system and share learning opportunities across health and disability services.
Te Tāhū Hauora guides and supports Aotearoa New Zealand’s health and disability services with a nationally consistent approach to reporting, review and learning from harm and near misses.
